I, ______________________________, hereby acknowledge that I have been fully informed of the inherent risks associated with getting a tattoo or piercing. I understand that these risks, both known and unknown, may lead to injury, including but not limited to infection, scarring, difficulties in detecting melanoma, and allergic reactions to tattoo pigment, metal reactions, latex gloves, and/or soap.

 

Despite being informed of the potential risks associated with getting a tattoo or piercing, I still wish to proceed with the application, and I willingly accept and expressly assume any and all risks that may arise from tattooing or piercing. I confirm that both the Artist and 510 Expert Tattoo have provided me with the full opportunity to ask any and all questions about the application of my tattoo or piercing, and all my queries have been answered to my total satisfaction.

 

I hereby WAIVE AND RELEASE, to the fullest extent permitted by law, each of the Artist and 510 Expert Tattoo from all liability whatsoever, for any and all claims or causes of action that I, my estate, heirs, executors, or assigns may have for personal injury or otherwise, including any direct and/or consequential damages. Such damages may result or arise from the application of my tattoo or piercing, whether caused by the negligence or fault of either the Artist or 510 Expert Tattoo, or otherwise.

 

I affirm that I am at least 18 years of age and do not have diabetes, epilepsy, hemophilia, a heart condition, nor do I take blood-thinning medication. I confirm that I do not have any other medical or skin condition that may interfere with the application or healing of the tattoo. Additionally, I am not the recipient of an organ or bone marrow transplant, or if I am, I have taken the prescribed preventive regimen of antibiotics required by my doctor in advance of any invasive procedure such as tattooing or piercing. I am not pregnant or nursing, and I do not have a mental impairment that may affect my judgment in getting the tattoo.

 

I acknowledge that neither the Artist nor 510 Expert Tattoo is responsible for the meaning or spelling of the symbol or text that I have provided to them or chosen from the flash (design) sheets. I understand that variations in color and design may exist between the selected tattoo art and the actual tattoo when applied to my body. I am aware that over time, the colors and clarity of my tattoo may fade due to unprotected exposure to the sun and the naturally occurring dispersion of pigment under the skin. Furthermore, I understand that if my skin is dark or tanned, the colors may not appear as bright as they do on light skin.

 

I agree to follow all instructions concerning the care of my tattoo or piercing and acknowledge that any touch-up work needed due to my own negligence will be done at my own expense. I also agree to promptly leave the premises of 510 Expert Tattoo upon request for any reason whatsoever by any employee of 510 Expert Tattoo.
